This session highlights Edinburgh HSCP’s transformation of its falls prevention pathway, developed through a 2.5-year, whole-system collaboration. By bringing together health, social care, leisure, and digital partners, the programme redesigned services to shift from reactive care to prevention.
The new approach integrates consistent screening, unified assessment, targeted interventions, and community-based activity programmes, supported by workforce development and digital tools. Early results show reduced emergency attendances and fractures, improving outcomes and system efficiency. The session will share key learning, impact, and how this scalable model can be replicated across other areas.
