Health and social care services across Scotland face growing pressures and must transform to meet future needs. The Scottish Approach to Change brings together a range of evidence-based quality improvement and change management methodologies into a single framework. It provides both a strategic approach for leading change across organisations and a practical toolkit for delivering improvement at local, regional and national levels.
In this session, Healthcare Improvement Scotland and the Centre for Sustainable Delivery will share their experience of leading and supporting large-scale transformation programmes that have redesigned pathways, improved access to care and delivered more sustainable models of service delivery.
Drawing on a range of national case studies, we will explore how multidisciplinary teams have successfully developed, implemented and scaled new ways of working. Topics from across NHS boards and health and social care partnerships across Scotland will include:
- expanding and embedding new professional roles, including Surgical Care Practitioners.
- supporting Type 2 Diabetes remission programmes
- Improvements to meet operational priorities in frailty outcomes
- mental health transformation in community services and delayed discharges
These approaches have supported improved outcomes for people, increased quality and safety, a more values-based delivery of care including more effective use of NHS resources contributing to reduced waiting times and delayed discharges.
The session will examine the key factors that enabled successful implementation, including clinician engagement, cultural change, collaborative leadership and the practical challenges of delivering sustainable transformation at scale. Participants will leave with practical insights into leading change, engaging clinical teams, scaling innovation and translating improvement into lasting impact across NHS Scotland.
