NHS Lanarkshire’s Interface team is leading bold, human-centred change to reshape care across the system. In the face of complex change, the team has remained grounded by their purpose of keeping people safe at home while building the foundations for a more responsive health system.
The team’s hard work and dedication is further strengthened by the Flow Navigation Centre+Plus (FNC+Plus) and Hospital at Home virtual wards – innovative services that reduce wait times, avoid hospital admissions and deliver personalised care tailored to individual patient needs.
Operating from a state-of-the-art control centre, FNC+Plus brings together nurses, emergency consultants, ambulance personnel and support staff. As the only service of its kind in the UK, FNC+Plus is already proving to be truly transformative, with thousands of patients benefitting to date.
